Closing the language gap in care documentation

Scandinavian municipal care is delivered by a workforce that speaks many languages. The documentation it produces - and the regulation it must comply with - are almost entirely in Danish or Swedish. That gap between the staff who do the work and the language in which the work must be recorded is the biggest single drag on care-documentation quality today. Clex is built for it.
The shape of the gap
- 42 % of Danish basic elder-care staff are foreign-born. Among social- og sundhedshjælpere the share is over 50 %. (EVA / Altinget 2024, SOU 2024:78.)
- 25 % of care staff need language support to meet the written demands of the job. (Äldrecentrum 2023.)
- Denmark alone will be short 15 000 care workers by 2035. Sweden projects an equivalent gap across äldreomsorg. (EVA 2024, Socialstyrelsen.)
These are not temporary staffing problems. The demographic arithmetic - more 80+ residents, fewer working-age adults, record-low recruitment of domestic care workers - guarantees that a growing share of Scandinavian care is delivered by staff whose first language is not the language in which records must be kept.
The risk that follows is not a recruitment risk. It is a care-quality risk. Ambiguity in a written note (was it blod, or blød afføring?) becomes a phone call, a missed observation, or a delayed response. The fix is not “hire differently”. The fix is to make the documentation itself easier to get right.
What Clex changes
Clex is a keyboard on Android and iOS, and a browser extension on Chrome and Edge. Instead of asking a bilingual care worker to compose a full Danish or Swedish sentence from scratch, Clex lets them:
- Type a few keywords - body area, condition, care situation - even in fragments or broken word order. Word suggestions above the keys - trained on care-documentation language, not general phone-keyboard vocabulary - help find the right Danish or Swedish term as the worker types. Clex reads the care concept behind each word and shows the matching pictogram beside it.
- Read and accept a suggested sentence in Danish or Swedish, properly phrased for the record system.
- Hear it read aloud by on-device text-to-speech before saving, to confirm the meaning.
- See a live translation into their own language, displayed beside the text and generated entirely on the device.
The pictogram carries the meaning. The Danish or Swedish sentence goes into the record. The translation confirms that the sentence in the record says what the worker intended. None of that depends on the care worker’s written proficiency. Nothing in that workflow sends free text off the device - only the pictogram identifiers and the chosen language are sent.
Twenty-three languages on the phone, twenty-eight in the browser
Clex Keyboard on Android and iOS ships with translation and text-to-speech for 23 languages:

Who this helps most on a shift
- Bilingual staff writing in their second language. The slowest, most draining part of the shift is now done quickly.
- Staff with dyslexia or other language-processing differences. Writing a few keywords and confirming a proposed sentence replaces writing from scratch. The same approach that helps a Somali-speaking home-care assistant helps a Danish-speaking staff member who struggles with spelling.
- Night-shift and weekend staff who do not have a colleague to cross-check a note. The on-device read-aloud gives an immediate second check.
- New hires in their first weeks. A new hire can learn the pictogram-to-sentence workflow in a single shift.
- Staff on personal devices via BYOD. Activation happens without IT intervention.
